There are forms which need to be completed prior to entry. A package of these forms is available at Aminya Village. Please feel free to pick up a package during office hours or contact us via email on firstname.lastname@example.org or by phone on 08 8569 1749.
If you are in need of care, or have a relative in need of care, there are some things that need to be organised first:
- ACAT Assessment
You can ask your Doctor or health care professional to organise the ACAT for you, or you can contact the My Aged Care contract centre direct on 1800 200 422. It is a free assessment, but you will need a My Aged Care client record to set up an assessment.
The Assessor will visit you where ever you are either at home or in hospital, and the process will take about 45 minutes to an hour and a half to complete. You can have someone of your choice with you during the assessment.
- Appointment with the Administration/Finance Officer
Once you have the necessary paperwork you may then make an appointment to come and meet with our Administration/Finance Officer, see our facility and discuss your needs. It is during this appointment that you will be given information about the availability of accommodation (as it constantly changes), along with the cost of accommodation. Please ensure you bring with you to this appointment a copy of your ACAT letter.
Application for Independent Living Units
To apply for Independent Living Units, you need to contact the administration officer on 08 8569 1749 at Aminya to obtain a Registration of Interest form.
